Abstract

At Kingston Hospital the Gamma nail has only been used for the fixation of unstable intertrochanteric and subtrochanteric fractures of the femur. There have been 28 cases; 21 required distal locking. Of these, there were intraoperative difficulties in nine.

There was also one case with osteosclerosis which also turns the bone fragile, in spite of the harder structure, because harder does not mean stronger [103]. Localized bone loss occurs as a result of preloading by the fixation device in the femoral neck, producing stress shielding, reducing the strain at the neck, and increasing bone loss and weakness [104–108]. Further factors favoring local fragility included back out of the implant from the head with reduction of the failure strength of the neck-head at the site previously reached by the metal [108], avascular necrosis of the femoral head [109] and subclinical osteomyelitis with secondary avascular necrosis of the femoral head.

Descriptive statistics were employed where possible. After omitting studies that did not fulfill the inclusion criteria or were repeated in the databases search, there remained 24 references for inclusion in the analysis [11–34]. All studies were retrospective.
