Cette étude présente l'évolution des taux de cortisol et de testostérone salivaires dans une population de jeunes lutteurs de haut niveau en centre de perfectionnement, d'entraînement et de formation comparée à des étudiants non sportifs de même catégorie d'âge. Les mesures sont faites au cours du premier trimestre scolaire à intervalles de 3 semaines. Elles font apparaître un niveau de fatigue plus élevé chez les sportifs par rapport aux non-sportifs. Les résultats mettent également en évidence qu'un repos de 2 semaines (congés de Toussaint) permet un retour des taux hormonaux aux valeurs de référence de la rentrée scolaire.
Summary
This study presents the saliva cortisol and testosterone levels in young high level wrestlers compared to those of students not involved in sporting activities. Measures were realised during the first school term (September to mid-December) at 3-week intervals. Hormonal levels revealed a higher degree of tiredness in sportsmen than in non-sportsmen. It also appeared that a 2-week resting period (All-Saint's Day at the end of October) permitted return to hormonal reference values at the beginning of the school year.

Position statement: contribution of the biologic analyses in the diagnosis of overtraining syndrome
2006, Science and SportsApporter au travers d'une position de consensus, un outil complémentaire à celui de l'examen clinique pour le médecin du sport, et connaître les indications et les limites de la prescription des examens biologiques de repos chez le sportif présentant des signes cliniques évocateurs de surentraînement.
Une position de consensus a été rédigée à la suite d'une revue de la littérature portant sur les aspects hormonaux, hématologiques, immunologiques et métaboliques des états de surentraînement et de l'expérience clinique personnelle des auteurs. L'examen clinique et en particulier l'interrogatoire préalable sont indispensables avant toute prescription biologique. Les conditions préanalytiques et analytiques de prélèvements doivent être connues et standardisées. Les règles de prescription des examens biologiques sont discutées pour répondre aux questions suivantes : pourquoi prescrire ? Quand et comment prescrire ? Quels examens prescrire ?
Il n'existe pas d'examen biologique pathognomonique de l'état de surentraînement. En revanche, certains examens biologiques nous paraissent pertinents pour réaliser le diagnostic différentiel et/ou caractériser les facteurs déclenchant ou favorisant cet état.
To provide a complementary tool to clinical examination for the sport physician and to determine the indications and the limits of the prescription of the blood analyses performedat rest in the athletes, who present clinical signs evocative of overtraining.
This position of consensus was established following a literature review on the hormonal, haematology, immunological and metabolic aspects, and the personal experience of authors. The clinical examination and particularly the medical interview are essential before any biological application. Pre-analytical and analytical conditions of sampling have to be known and standardized. The rules of biological analysis prescriptions are discussed in order to answer the following questions: Why to prescribe? When and how to prescribe? Which examinations to prescribe?
There is no pathognomonic biochemical analysis relevant to the state of overtraining. However, some of several biological analyses appear relevant to us to carry out the differential diagnosis and/or to characterize the triggering or favouring factors of overtraining.
Do circannual rhythm of cortisol and testosterone interfere with variations induced by other events?
2006, Annales d'EndocrinologieCortisol et testostérone sont deux hormones dont les taux varient en réponse à diverses situations sportives ou professionnelles. Il est légitime de s’interroger sur l’influence éventuelle d’un rythme circannuel avant d‘affirmer que des changements dans le niveau de ces hormones sont imputables à une cause unique.
Pour le cortisol, l’étude transversale a été réalisée sur 102 hommes adultes (42 ans) sur des échantillons de salive prélevés une demi-heure après le lever. Les valeurs ont été regroupées par trimestre correspondant aux saisons. Pour la testostérone, les conclusions sont tirées uniquement de l’analyse de la littérature.
La moyenne des taux de cortisol sur l’année est de 14,36 ± 0,44 nmol.l- 1. Aucune différence significative n’apparaît entre moyenne et valeur pic ni entre valeur saisonnière maximale et minimale. Pour la testostérone, il existe un nombre limité d’études qui ne sont pas unanimes. L’amplitude des variations est de toute façon faible (9,7 % entre pic et moyenne annuelle) et est en partie imputable à la variabilité individuelle et inter-individuelle.
Notre étude conclue pour le cortisol à l’absence de rythme saisonnier (ou circannuel) dont l’importance pourrait interférer avec les effets provenant d’une autre cause. Pour la testostérone, le rythme circannuel peut rendre compte de 10 % de la variation.
Cortisol and testosterone are two hormones whose levels may vary in response to sports or occupational events. We wondered if the circannual rhythm of these hormones could have an influence on such responses or whether changes can always be ascribed to a single cause.
For cortisol, we conducted a cross-sectional study among 102 adult men (mean age 42 years) using saliva samples taken one half hour after awakening. The values were combined over three-month periods corresponding to the four seasons. For testosterone, conclusions were drawn from data reported in the literature.
The mean annual cortisol level was 14.36±0.44 nmol/l. There was no significant difference between average and peak values nor between maximal and minimal values. For testosterone, there have been a limited number of studies and it is unclear whether there is a seasonal change. In any case, the amplitude of variations is weak (9.7% between peak and annual average), which is partly ascribable to individual and interindividual variability.
We conclude that there is no seasonal (or circannual) rhythm in cortisol levels to a degree which could interfere with effects resulting from other events. For testosterone, the circannual rhythm may account for 10% of the variation.
Biological markers for the follow-up of athletes throughout the training season
2004, Pathologie BiologieDuring the training season, a state of fatigue known as overtraining may occur, resulting from an excessive load of training, both in volume and intensity. Even now, difficult to predict the risk of overtraining, although this syndrome has been the subject of numerous studies. A lot of biological markers have been propounded. Taken alone, none of them have an absolute significance. This paper aims to review these markers, considering their biological interest, the ease with which they can be measured and the cost, from the simplest (body weight daily recording) to the most up to date markers (e.g. anti-oxidant status). They are grouped into three categories: non-invasive behavioural and biological markers, biochemical markers, and hormonal and immunological markers.
Au cours de la saison d’entraînement et de compétition chez le sportif, un état de fatigue nommé surentraînement peut survenir. Il est le résultat d’une charge excessive d’entraînement, à la fois en volume et en intensité. Malgré que ce syndrome ait fait l’objet de nombreuses études, il est encore difficile à ce jour de prédire le risque de survenue. Un grand nombre de marqueurs biologiques ont été proposés, mais aucun d’entre eux pris isolément n’a de signification absolue. Cet article a pour objet de recenser ces principaux marqueurs, en prenant en compte leur intérêt biologique, la facilité et le coût de la détermination, en allant du plus simple (pesée quotidienne) au plus élaboré (statut anti-oxydant par exemple). Ces marqueurs sont regroupés en trois catégories : marqueurs comportementaux et biologiques non invasifs, marqueurs biochimiques et marqueurs hormonaux et immunolmogiques.
The French Society of Sport Medicine comportmental questionnaire. Correlations with cortisol and testosterone levels throughout the sporting season in a high level rugby team
2003, Science and SportsObjectif. – Réaliser le suivi de 21 joueurs de rugby au moyen du questionnaire français de la Société française de médecine du sports (SFMS), des taux de cortisol (C) et de testostérone salivaire (T), et de leur rapport T/C.
Matériels et méthodes. – Les joueurs ont répondu au questionnaire d’état de forme de la SFMS qui permet d’attribuer un score proportionnel à l’état de fatigue. Ces sujets ont fourni trois échantillons de salive au lever (8 h), avant le déjeuner (11 h), et à 17 h afin de réaliser les dosages hormonaux. Les premières mesures ont été réalisées avant le début de la saison (T1), puis à 12 semaines (T2) et 24 semaines (T3) après T1. La performance de l’équipe est déterminée à partir du nombre de victoires sur la saison.
Résultats et discussion. – L’augmentation du score obtenu au questionnaire d’état de forme de la SFMS et les variations hormonales au cours de la saison semblent traduire un phénomène normal d’adaptation à l’entraînement, ce qui est confirmé par le maintien du niveau de performance de l’équipe. Cette étude permet également de mettre en évidence les évolutions parallèles entre l’index psychocomportemental (score) et les marqueurs biologiques de l’état de forme. Ainsi, il apparaît que le questionnaire SFMS est un outil intéressant et facile d’emploi pour évaluer l’état de forme d’un sportif au cours de la saison.
Objective. – To carry out the follow-up of 21 high level rugby players by means of the psycho-comportmental questionnaire of the “French Society of Sports Medicine” (SFMS), of the salivary testosterone (T) and cortisol (C) levels and of their ratio T/C during the sporting season.
Materials and methods. – The players provided three samples of saliva, at awakening (8 am), just before the lunch (11 am), and at 17 pm in order to assay cortisol and testosterone. The first measurements were done at the beginning of season (T1), 12 weeks (T2) and 24 weeks (T3) after T1. At the same periods, they completed the 54 items of the SFMS questionnaire which allows to give a score (from 1 to 54) proportional to the state of tiredness of the subject. The performance of the team was estimated from the ratio of victories to the number of matches played over the season.
Results and discussion. – The increase in the score obtained with the SFMS questionnaire and the variations of the hormonal levels during the season seem to reflect a normal adaptation phenomenon to the training load, a fact which is confirmed by the maintenance of the same level of performance of the team throughout the season. This study permitted to highlight the parallel evolutions between the psycho-comportemental index (score) and the hormonal markers of the state of tiredness. Thus, it appears that the SFMS questionnaire is an interesting and easy to employ tool to evaluate the state of tiredness (or reciprocally of form) of a sportsman during the course of the sporting season.
