Inspiration occurs when the contraction of inspiratory muscles (namely, the diaphragm and the external intercostal muscles) makes the pleural pressure be lower than outside the lungs (i.e., sub-atmospheric), therefore, creating an airflow entering the lungs that expand together with the chest wall. If the opposite occurs, then air rushes out and expiration occurs [3,4]. The amount of air moved through the lungs with each respiratory cycle is tidal volume.

Inspiratory muscle fatigue is defined as a reduction in the maximal power-production capacity of the inspiratory muscles (Coirault et al., 1995). The causes of this fatigue are increased ventilation load, acidosis, decreased arterial oxygen content, decreased blood volume in the inspiratory muscles, and decreased energy stores in the inspiratory muscles (Johnson et al., 1996; Macklem, 1980). Inspiratory muscle fatigue is an example of peripheral fatigue.

However, presence of daytime hypercapnia has been considered a factor for poor prognosis in the outcome of COPD patients who require HOT.138–140 Hypercapnia may enhance muscle dysfunction, reduce diaphragm contractility and lead to muscle fatigue.141,142 It may also influence cardiac contractility, leading to arrhythmias and structural injuries in the myocardium.143–145

These mechanisms suggest an additional potential benefit from inhaled beta-2 agonists in the severely dyspneic patient. In the patient with increased work of breathing, the oxygen demands of the respiratory muscles may comprise a significant fraction of the total oxygen supply to the body (34). Viires et al. have shown that in spontaneously breathing animals with a low cardiac output produced by pericardial tamponade, the respiratory muscles received 21% of the cardiac output, as compared with 3% when the animals were paralyzed and ventilated (35).
