Plication of the anterolateral capsule of the ankle with extensor digitorum brevis transfer for chronic lateral ligament instability

Abstract

Thirty sportsmen and women with chronic sprain of the lateral ligament of the ankle characterized by instability and pathological talar tilt have been treated by plication of the anterolateral capsule with extensor digitorum brevis transfer and all have returned to normal sporting activity.

    The sharp edges of the ATFL are imbricated with small nonabsorbable sutures, keeping the repair knots small and between soft tissue layers. The extensor retinaculum has been used and the Gould modification has been performed in the presence of CFL insufficiency.5,6,27,28,44–47 Rehabilitation following the Brostrom procedure incorporates immobilization and limited weight bearing, with a protection period of 3 months before an increase of activity.
