Background
There are few scientific prospective studies, and few studies that compared different types of conservative management regimens for the chronic painful Achilles tendon in a randomized manner. Conservative management is recommended as the initial strategy [1], [2], [4], [6], [8], [9], [10], [18], [19], [20], with identification and correction of possible etiologic factors, at times using a symptom-related approach.

Summary
Chronic Achilles tendinopathy is difficult to treat, and results, even after surgical management, are variable. The few studies that reported long-term results indicated a poor outcome. Also, surgery requires prolonged rehabilitation, and, depending on the patient's occupation, a varying period of sick leave from work. For tendinopathy of the main body of the Achilles tendon, painful eccentric calf muscle training gave good short- to mid-term results, and does not require taking sick leave from
